Information for AUBMC patients

This notice is intended for patients who were previously referred to Dr. Marc J. Haddad through AUBMC or who received care from Dr. Haddad while he was practicing at AUBMC.

Continuity of care

Please contact AUBMC to schedule with another provider.

Following Dr. Haddad's tenure with AUBMC, there is an agreement between the provider and the hospital restricting competition and the continued care of previous patients who were referred to Dr. Haddad at AUBMC. For that reason, Haddad Gastroenterology is not able to continue care for patients referred to Dr. Haddad through AUBMC.

AUBMC patients should contact AUBMC directly and request scheduling with another appropriate provider within AUBMC. This is the correct pathway for continued hospital-based care, follow-up appointments, and transfer to another physician at AUBMC.

Medical history and records

Requesting your records or forwarding information.

In accordance with applicable law, only the providing physician retains patient records. Records relating to care provided by Dr. Haddad were handed to him following his tenure at AUBMC. AUBMC will not have a record of your presence with Dr. Haddad for this care.

In accordance with the Electronic Transactions and Personal Data Protection Law (Law 81/2018), patients are allowed to request records from the providing physician directly. If a patient wants information forwarded to a new provider at AUBMC, or wants to request records directly from Haddad Gastroenterology, the request may be sent by email.

Please note that record requests can take up to 45 days to process, excluding religious and/or national holidays. If you have found another provider, please contact that provider's records request department so the request can be directed through the appropriate channel.

  • Name of the new provider or department, if records should be forwarded.
  • Whether the request is for a copy of records or transfer to another provider.
  • Any specific date range, report, laboratory result, imaging report, or consultation note requested.
